Front End is not straight??!

The Alignment of my front end is off... The PO had a crash because the headlight brace is welded to the frame entirely.  When I'm riding and the wheel is straight, the handle bars felt off- the right side would be more in than the left. the bottem of the top triple has the notch or the point and it should be facing straight to the frame but its not. It feels out of whack and The big nut on the top was loose when I bought the bike. We remedied it by holding the wheel and pulling it straight and tightening it back up but it seems to be out of whack again... kinda pisses you off when you ride ya know?
