problem with clutch

when i accelerate in any gear,any rpm initially for 3/4 sec the rev shoots up and clutch gets engaged and after a couple of sec the rev drops ......then accelerates smoothly.This happens in every gear when i accelerate.what may be causing the clutch to engage.
and does anyone know the spec of front fork brace allen screws,noticed today 2 of the screws have come loose and fallen off,would this cause the bike to be dragged to one side(yesterday felt like had a flat tire but after few meters the bike became steady).

Before condemning the clutch make sure the cable is adjusted properly.

Did oil change about 2000 miles back,use 10w40 regularl oil and oil level is optimum .Dont know what grade oil previous owner had used before this oil change.
