How to replace chain adjuster/tensioner

You can remove the back plates and pound them flat.
Insure they stay in place as you tighten the chain.

If you want to replace the whole chain adjuster things, remove the axle and pull them out.

When you say take the the back plates off- do you mean when the axel nut is tight on the bike and just loosen the 12mm adjusters and take off the back plate?

Remove the nuts on the end of the adjusters.
Slip off the plates.
Pound them flat and put back on. The rectangle end plates can slip out of position then get bent when you tighten the chain adjuster.
No need to remove the axle to take off those back plates and adjuster nuts.
Axle will stay in place. The adjusters are there to pull the axle backwards as you tighten the nut, in order to tighten the chain and align the wheel.

FYI: To move the wheel forward to start the adjustment all over, you loosen the adjusters, loosen the axle and give the wheel a swift kick to move forward. Then you start the adjustment all over with the adjusters to pull the wheel backwards.

FYI #2: When you tighten the adjusters, the axle needs to be loose. The adjusters only pull the wheel back. They are not there to keep the wheel in place. Tightening the axle keeps the wheel in place. Do not tighten the crap out of the adjusters with the axle tight - you'll just bend in the back plates.

and make sure you do each side in turn ie 1/2 turn left, 1/2 turn right, rinse and repeat.  that way the adjustments are even each side.
