Part number SU-5589.  I bought it from a forum member and ended up going back to the stock filter.  The filter is in good shape, might need oiling.  I don't have the restrictor, but it can be purchased from K&N customer service per this thread: http://gstwins.com/gsboard/index.php?topic=58037.0 .
