the in and out burger was popularized by the movie "the big lebowski" to some extent. i believe that they are soley west coast based, as i've never seen nor heard of one anywhere else.
There are In & Outs here in Phoenix and in Las Vegas and people seem to get fanatical about them. I have never eaten there so I can't comment on the food. The name is most assuredly unappealing.

probably the best fast food burgers in town (IMHO). I love them dispite the grease, and you know the grease is probably what makes it taste so good.
The stuff is never frozen (its shipped properly but never freezer burn kinda thing). It actually tastes different, tastes fresh. You can order the burger in Animal Style (mustard in the patty meat, extra lettuce, extra sauce, grilled onions and something else). You can ask for your fries well done (normally fries are soft). Their shakes tates better than MC'D's.. but not JIB. For the protein fanatics they offer protein style (no buns, its wrapped in lettuce).
Basically all they serve is burgers, fries, soda and shakes. Nothing else. It takes a while to get your food, but everything is freshly prepared and hot off teh grill.
I love the place.. mmmmmm... was a good lunch there i tell you.

every time I'm in Cali. I make sure I eat at In 'n Out at least three times. Best fast food burgers you'll ever eat. The most I've done is a 4x4. (four patties.) that guys is crazy. but check this out...

that's my friend matt (rides an '82 csr1000) in the red shirt. In front of him is the Logger Burger. One full pound of beef. biggest burger I've ever seen... until now.
