I set the rear brake lever as high as possible, but I've always felt the angle is too steep.

This is a foot from an amplifier or guitar cab. It's chrome with a solid rubber cushion and a through bolt. It works well and I like that I can off-set the position.
The rounded edge doesn't catch on your boot, so you can just pivot to from a resting position..
The grip surface is just a piece of sticky-back sandpaper for now, I'll have to find something more durable, (maybe that grip-tape they put on wooden stairs?)
For me this makes a huge difference in slow speed situations, tight parking-lot maneuvers, stop and go traffic etc.
