I have a rotor, no idea about miles, but its in good shape with no discernible or measurable grooves or wear steps etc etc - How's $25 and shipping.

I have a set of controls, gonna be about a week until I can ship them out though, $20 shipped would take them.

These aren't that great ... they not grooved, but they are right at 4mm ... in fact I never seen one over 4mm ... maybe that's the thickness new ... who knows ... anyway 2 bucks for rotor, $5 for labor and 12 shipping ... OK ... yea paypal takes $1 ... or buddha cannot add  ... whatever ...

You have a front brake hand lever laying around anywhere?

Since I have no clue what its original thickness is, and how many miles it ran before it went to under 4mm and the fact that it says 4mm minimum on it means ... bleeeeeeh ... no clue.
All I can say is ... I measure them ... and tell you what their thickness is ... it a shade under 4 ... like 3.96 ... I have run em to 3.5 in fact ... and I dont think they are much over 4mm new ...
4mm may be some DOT crap ... to cover their ass. Or maybe the magic number ... dunno
Anyway ... I sorta refrain from selling crap like this ... but I dunno my preaching kat FE at anyone with a pulse seems to have gone on deaf ears ... so you guys can deal with crap some of the kat swaps left over in my house ... OK fine make it 15 bucks. It prolly gonna be 12 bucks to mail it.
